Learning Model Survey

Fill out one survey for each child.

Students will have the option to continue distance (at-home) learning or begin attending on-campus (face-to-face) learning beginning Oct. 5. The decision to continue distance learning is binding through the end of the grading cycle, which is Nov. 6.

Information you will need:

• Your child's student ID number
• Your child’s current school
• Your child’s name (last, first)
• Your child’s date of birth (mm/dd/yyyy) month/day/year
• Your child’s current grade level

Choose the school for your child from the list by grade level below. Remember that you need to fill out a separate survey for each of your children
